Faculty And Staff Flower Fund

NOTE:   When a member of the CHS Faculty and Staff learns about an illness or death, he/she should notify the principal’s office.  While this method may produce duplicate reports, that would be better than missing a situation altogether.

• FUNDING

It is proposed that each member of the CHS faculty and staff donate $5.00 to be taken up on or before the October faculty meeting. Each department head will be responsible for collecting from his or her department.

ILLNESS/SURGERY
Gift cards shall be sent when a member of the Faculty or Staff is hospitalized. (Estimated amount will be $15.00).

• MEMORIALS

1. Either floral arrangements, memorial books or memorial contribution shall be given upon the death of the following:

1. An active Faculty or Staff member
2. A member of the immediate family (husband, wife, child) of an active Faculty or Staff.
3. The father or mother of a member of an active Faculty or Staff

• Sympathy cards will be signed by CHS Faculty and Staff in the event of the death 

of the following:

1. Either parent-in-law of an active member of the Faculty and Staff.
2. The brother or sister of an active member of the Faculty and Staff.

FLORAL ARRANGEMENTS:  Flowers shall be sent unless otherwise requested.  (Estimated expenses $35.00). 

MEMORIAL BOOKS:  Upon request, an appropriate book shall be placed in School Library.  (Estimated expenses $35.00).

MEMORIAL SCHOLARSHIP CONTRIBUTIONS:  Upon request, a contribution shall be made to the CHS Scholarship fund.  (Estimated expense:  $35.00).
