Student Incident Report
In an effort to ensure safety of our students and the best use of Cookeville High School's resources, if you observe any student engaging in any activity which you consider to be illegal, harmful or improper, please submit that information using one of the following three forms. Your submission will go directly to the Assistant Principal who will investigate the incident. Your identity will NOT be divuldged and will be held in confidence.
Please complete the form properly and in full detail of the incident listing names, dates and specific details of the incident that has occured. Any false and/or inappropriate incident reports will be investigated and could result in criminal charges.
