CHS Athletic Physicals

On Tuesday, May 25, 2010, pre-season physicals for all athletes in grades 9-12 at Cookeville High School will be held in the CHS gym. The cost for the physical will be $15.00. All of the proceeds will be returned to the Sports Medicine program at the school. These funds will be used to purchase supplies needed for all athletes. The TSSAA has mandated that all athletes have a pre-season physical each school year. Once the current school year is complete in May, the physicals are no longer considered current. For this reason, all athletes must complete a physical each year before the athletic season. Should athletes be unable to attend on May 25th, he/she will need to have a physical completed by a physician before the start of the athletic season and turned in to the appropriate coach, who will in turn, pass it on to the athletic trainer, Brent Ryan. There WILL NOT be a makeup day for physicals. Athletes will not be able to practice or compete until he/she has a physical completed and on file in the athletic trainer’s office. This includes sports activities over the summer (Camps, weightlifting and conditioning). Athletes must see his/her respective coach to get the Sports Medical Form and History/Release Forms.  You may also pick one up in Mrs. Winchester’s office. These forms muse be completed and brought back with athlete at the time of the physical. This is the athlete’s “ticket” to get into the physical. Our physicians cannot conduct your physical without these completed forms signed by a parent or guardian. We will be providing an orthopedic exam, so we ask that each athlete wear shorts and a T-shirt. Please remember that it is hard to examine your knees through a pair of blue jeans.
